All cytogenetic testing, molecular genetic testing and DNA testing requisitions must be faxed to MLW – Client Services Department at 519-258-9505 by the physician prior to the patient’s blood draw.
MLW will contact the patient to schedule an appointment. Please ensure that the requisition contains the patients updated contact information.
